Long term, affordable rental housing for people with disabilities who have been homeless or are at risk of becoming homeless. Ongoing case management services are available.

Countywide (Santa Rosa location)
Buckelew Programs: 571-5581.
 Family Service Coordination, housing, independent living skills, and employment services for adults with mental illness. Family Service Coordinator (FSC): free education, support & assistance with accessing community resources & navigating mental health system for anyone with an adult family member or loved one with a mental illness. Call FSC at 571-8452 for info or come to Resource Clinic onsite every Tues at 3pm, no appointment necessary. Supported Housing / Independent Living Skills: by referral from Sonoma County Behavioral Health (must have SCBH case manager). Supported Employment: orientation onsite every Tues & Thurs at 2pm, no appointment necessary. Catholic Charities Permanent Supportive Housing (Section 8): 542-5426.  For very low income families & individuals. M-F 9am-5pm.

Community Action Partnership Housing Program: 544-6911. For homeless women with children. Case management, parenting classes, permanent housing referrals, support groups. M-F 8:30am-5pm. Community Support Network (CSN): 575-0979. Residential programs for transitional age youth and adults with a mental health diagnosis &/or behavioral health challenges. M-F 9am-4:30pm. (Sonoma Co. Reach for Home: 433-6161.  Helps participants to work toward independence & permanent housing, access resources & services, develop good credit & rental history, address substance dependency issues & gain self-confidence.  Referrals are processed through Coordinated Entry.
